Graduate School

• Official copy of the transcript of records and a certificate of honorable dismissal of transfer credentials from institutions of higher learning last attended.
• Permit to study (if employed)
• Service record
• Certification of English Proficiency by a professor of English (only for applicants from the foreign countries where English is not a medium of instruction and or not the native language.
• Letter of recommendation from two (2) former professor and immediate supervisor.
• For MPA applicants, must have at least 1-year work experience.
• For Ph.D. applicants, must have at least 2-year of teaching or administrative experience.

Undergraduate

• Must be a high school graduate
• Must pass the university and school/college written admission/entrance examination
• Must satisfactory pass the interview
• Must submit the following credentials

For New Students

• Form 138 (report card) or transcript of records (a GPA of 85% or higher for teacher education course and grade of 85% or higher in Science and Technology, English and, Mathematics for DVM course required)
• certificate of good moral character signed by school head
• medical certificate signed by a government physician
• certified true copy of live birth
• two recent ID pictures (1" X 1")

For Transferees Certified true copy of transcript of records
